Rates:  Security/Damage Deposit:  $120.00
    Rental Fee:    $120.00 / 24-hr period for groups of 25 or less
                             ($150 after 1/1/03)

                    $150.00 / 24-hr period for groups of 26 or more ($175 after 1/1/03)
                    $700.00 / 7 consecutive days
        Note:  These rates apply to day usage as well
    Any waiving of these rental fees is at the discretion of the Temple Hills Board.

Lodge reservations and payments of rental should be made at the Methodist Temple.  This reservation is subject to cancellation by the Temple Hills Board anytime prior to three months in advance.  Reservations will not be confirmed more than six months in advance.

The $120.00 security/damage deposit must be paid in advance to hold a reservation.  No reservation will be final until this deposit has been received.  This deposit will be refunded after rental less any costs of repairs or damages (or deposit retained and additional charges to be billed if necessary).   Deposits will be refunded if scheduled activity is cancelled four weeks prior to scheduled date.  Any cancellation less than four weeks will result in forfeiture of the minimum rental fee of $120.00.

A Check-out report for calculating your fee and determining the condition of the facilities is enclosed with this reservation form.  Use the Check-out report when you arrive to be sure the center is as you expected it to be and as the caretaker reports it to be.  Use it again when you leave so that you will know whether or not damages have occurred while you were there.  Damages will be assessed for breakage, for failure to clean up properly, and for materials and equipment left in areas other than where they were upon arrival.  You will be billed actual costs of repairs to items damaged by your group if the costs exceed the $120 deposit.  Return the report within three days after the close of your retreat.
